what is the procedure for registration of service tax for multiple services ?
if any new service is provided after the registration then does the service provider have to register again?
any survey conducted comes under which category of services?

You can mention more than one service at the time of registration.
However, at a later stage you need to inform the department with the services included by you.

Yes. Submit ST-1 for the new service. Surrender the ST-2. Get fresh ST-2 incorporating all services from the department.
