Mismatch in name as per income tax record ?

05 April 2012 following is printed on the pan card

"MALLICK MOHD ARIF

IMTIYAZ ABUMOHAMED MALLICK

20/03/1985
permanent Account Number
ANBPM4461H"

I was filling DIN form. i tried entering various comnination in first name, middle name and last name and when i click on verify pan details the following is displayed.

"applicant name doesnot match with the record as per income tax database. kindly provide name as per income tax database"

what should be done to know what is the first name, middle name and last name entered in the pan application form.

i tried to get it through incometax website but server was down.

urgently waiting for your reply.
