Dear Sir,
If I receive 6 months of consultancy fees on which TDS is deducted @10% and the rest 6 months as salary, what are the amounts that I declare while filing IT return? Do I declare both my consultancy fees as well as my salary?

Yes. You need to declare all forms of incomes. Use ITR-3
