We have recd telephone bill but they have not mentioned our GST no and the same was not showing in GSTR 2A.

Shall we have to show that entry in ineligible input credit or how to pass the entry?

24 October 2017 You do not need to show it in 3B however if you wish can show it in Ineligible ITC in column "Others ".

25 October 2017 Ask the dealer to add your GSTIN in their invoices. You can upload the missing credit in GSTR 2 and ask the party to accept the credit.
