Incorporation of private company limited by shares

When we e-filed form INC 34, the form sent for re-submission because of the following deficiencies
"Definition of private company as per sec 2(68) and name of first director to be inserted in AOA."
Please clarify the deficiencies mentioned by them.
What changes should be made while re-submitting the form?

In the e-AOA
+ in clause 58 mention about the first director's of the company.
* include section 2(68) as such as clause 89 under the heading'others'
Then resubmit the form.

How to remove extra Table from MOA(INC -33).Table A is applicable to us. We select the Table A correctly.
