How to surrender additional pan allotted to individual

31 August 2016 Dear Sir
An Individual person allotted 2 PAN inadvertently. Now he want to Surrender additional PAN which is not used yet.
Please let me know following:-
1) is this possible to surrender Online mode or i have to surrender only offline, which mode is better? If it is possible through online mode please guide me website please.
2) should i have to upload or post any documents for surrendering PAN?
3) Any charges i have to pay for Surrender?

31 August 2016 You may choose any one of the following options to surrender additional PAN/s:-

a. Visit Income Tax Department’s website link http://incometax.sparshindia.com/pan/newPAN.asp and submit the required details.

b. Alternatively, you may fill and submit PAN Change Request application form by mentioning the PAN which you are using currently on top of the form. All other PAN/s inadvertently allotted to you should be mentioned at item no. 11 of the form and the corresponding PAN card copy/s should be submitted for cancellation along with the form.

31 August 2016 when using this site http://incometax.sparshindia.com/pan/newPAN.asp it is confusing that In 3rd compartment of form i.e. Name of the PAN holder*
Last Name/Surname :..................
First Name : ..............................
Middle Name : ..............................
Date of Birth/Date of.....................
Incorporation(mm/dd/yyyy) ......
Father's Name#: ......................
In above field Which PAN card details will be provided, i.e. PAN card which i have to retained or which i have to surrender????
