How one should prepare this fund utilization report, what type of transaction to be recorded ,Is it prepare on accrual basis, what is the format of this report if it is preparing for a company

preparation of such reports depends on the objective and instructions of the receiver of the report.
If this report is being made for the lenders, you need to prepare the utilization report on cash basis as the lender is interested in whether or not the drawings made have been utilized for the stated objectives or not.
