please provide me the formate of letter to send the shareholders in the matter of consolidation of shares of listed company giving option to them to convert their physical holding in demate and treatment of fractional shares.

Instead of sending a separate letter, you can send it in the notice of General meeting as well in notes section.
This will save the cost of Company.
