Form 32 - retirement of director

in case of filing form 32 for a director who retires in the AGM and does not get reappointed, while filing form 32, do we need to attach any document or only details in the form itself are sufficient for ROC submission?

please advice.

25 October 2012 "RESOLVED THAT Sakshi Sharma, holder of DIN 01780628 , who retires by rotation be not re-appointed and that the vacancy thereby created be not filled up and that the number of directors be reduced accordingly."

25 October 2012 thank you, sir. but the vacancy has been duly filled by another director whose nomination was submitted under 257.

so, does the resolution gets amended to :

RESOLVED THAT sakshi sharma, DIN........, who retired by rotation be not reappointed and that teh vacancy thereby created be filled up as per the necessary provisions of teh companies act and Articles of Association of teh company.
