What is the proof of "Constitution of applicant" have to be submitted to with ST-1 for application for Service tax Registration. If possible provide me the format.

its regarding the status of business identity, i.e proprietorship, partnership firm, pvt limited company, trust , LLP or any other, and you have to provide documentary evidence for each catagory in support of name used.
