Once I deposit the cheque into the bank and collect the acknowledged copy of the challan, do I have to file the challan with the ESIC department on month on month basis like Provident Fund.

If yes
a) what is the monthly remittance form #.
b) Can someone please forward the challan.
c) Now that monthly remittance is online and challans are generated in duplicate copy only should I submit my original copy to the department.
d) Some say filing is to be done on half yearly basis.If yes, please provide the form for the same.
