Declaration certificate under raid or survey

29 January 2015 In FY 2012-13 Income tax dept has visited my client place for surve / raid. Dept. has taken declaration from the assessee. One copy of declaration has been handed over to the assessee.

Now the case has been taken for scrutiny assessment for the year i.e.FY 2012-13. Assessing officer is asking for copy of declaration given to the dept. during the survey / raid.

However client has misplace that declaration.

Now my question is from where i'll get that declaration statement from the dept.
To whom should I apply.

29 January 2015 you need to apply from those officials who visited the assessees premises for raid
