A PERSON WAS IN EMPLOYMENT ,HE HAS EARNED 60000 AS SALARY INCOME FROM APRIL 2012 TO JUNE 2012,AFTERWARDS HE HAS CHANGE EMPLOYMENT ON PACKAGE OF 5 LAC

while computing the tax liability with your new employer, you inform about of income earned from previous employer.
In case, you were not informed to your new employer, don't worry. Show the income earned from your previous employer along with your current employer in your ITR 1 and pay the balance tax on or before 31st july.

In ITR 1, you add as many employer you want.

i want to know like form 16 ,whether there is any form for reporting

You can collect form 16 from both the employer. Anyway the credit will reflect in your form 26AS
