First you send request letter to ICAI to transferring of Articles with the reason to transferring of Articleship after getting permission from ICAI you have to send form 109 for the termination of Articleship. After getting the termination accepted letter from ICAI you have to send form 102 & 103 with new employer for the re-registration of your Articleship for the remaining period.
