16 February 2015 dear sir,

we filed MGT 14 for change of objectives of the company.But it came for resubmission with reason that provide undertaking to effect appropriate change in name to reflect proposed activities within 2 months of change in objects.can you please send me the format affidavit for this regards.

16 February 2015 I ,POOJA, aged about 25 years, Daughter of Mr.PANKAJ SHAH , Managing Director of AFFIDAVIT COMPANY PRIVATE LIMITED (here in after called as 'the company') solemnly declare and affirm as follows:-

THAT I am conversant with the facts of this affidavit and competent to make this declaration.

THAT the company has changed the objectives from 'manufacturing of chemicals' to 'trading in food products' more particularly described in the special resolution passed in the members meeting held on February 06,2015.

THAT the company had filed e-form MGT-14 with the Registrar of Companies, Mumbai to give effect to the special resolution.

THAT the Registrar of Companies,Mumbai has put up the said form for resubmission with a direction to file an undertaking to the effect that the name of the company shall be changed in accordance with the object of the company.

THAT, I do hereby declare that the name of the company shall be changed in such a way that the changed name will reflect the objective of the company with in 2 (two) months of the change in the objects.

I, POOJA, the above deponent do hereby declare and verify that the contents of this affidavit are true to my own knowledge and nothing material has been concealed and no part of it is false.
