Which form submit by bank to income tax after taking 15g fro

I want to know which form bank submit to income tax after collecting 15G or 15H form from client. & from where i can download it.

10 October 2013 Depends on the age of the client. If he/she is below sixty years, then Form 15G applies. If he/she is above 60, then form 15H applies.

10 October 2013 banks send a copy of the Form received from client to tax department.
