how the ledger scrutiny should be carried out? what are the main areas where it should be stressed?

Check whether the closing Balance of The ledger is as per its nature or not. The main areas are all, but more importantly loans & advances and other liabilities as most of the wrong grouping is in those areas only. Also narration plays a very important role as during ledger scrutiny you depend on the naration only.
Also check for TDS compliances in expenses i.e. TDS deduction and under the proper section.
