you need to submit request for approval for transfer alongwith medical certificates. you final attempt gets affected if you have excess leave resulting in shifting of completion of articleship beyond 30 april 2015. else your attempt remains same.

Can I take a certificate from government doctor confirming disease and ask him to state in his letter that you need quality and home food and clean surroundings to decrease the effect of your health problem. Is this way will helpful for my attempt not to postpone to may 2015

well that is for your doctor to decide. As I see it you are just trying to get a doctored certificate from a doctor to circumvent ICAI regulations.
