What documents require to be collect from employee for giving deduction U/S 80DDB (Rule 11DD)?

80DDB deduction is admissible for self or dependednt or a member of HUF for medical treatment of specified dieseases. Deduction not to be allowed by the employer/DDO. The employee should avail the concession by filing his/her IT returns before the assessing officer.
