if anyone is having cancellation form for service tax,or procedure for cancellation of service tax no please mail me!

There is no seperate form.
You have to write a letter informing your service tax officer about the closure of that service.
Till the date of clousure whatever be the dues take a demand draft.
Original certificate also has to be surrendered.
If you submit these 3 documents they will give the acknowledgement.
This is the procedure for cancellation.
