ANYONE CAN GIVE ME THE PRACTICAL EXAMPLES OF QUALIFIED REPORT, ADVERSE REPORT & DISCLAIMER REPORT??????

dear mam..
actually for one of my client factory got fired in june and all books of accounts lost by fire..
so there is no incomeplee records..
i have collected some information to deal this situation and i yet to finalise this
in this situation we have to disclaimer of opinion as books of accounts not available
