09 December 2014 I am filing form for inclusion of the company name for imparting Industrial training in EIRC of ICAI. There is an option in the form asking for address of Corporate Office. I am having doubt regarding this bcz the Company's Head Office is in Mumbai and Kolkata is only its Branch Office. So, here I should mention Mumbai address or Kolkata address? but as I want to empanel the company in EIRC so what address I should mention in that field?

Please clarify my doubt by giving your valuable opinions.
