representation before it authorities

please, can you post me the format of authorisation to appear before the income tax authorities, under section 288 of the income tax act ? what is the value of stamp paper on which the authorisation should be ?

BEFORE THE ASST COMMISSIONER OF INCOME TAX.MUMBAI
We abc ltd having our regd office at 12/20 m.g road,Bombay hereby appoint m/s xyz &co, chartered accountants having their office at 10/45, k.k road ,Bombay to represent us in connection with our incometax assessment for the assessment year 2007-08 and to produce and take back books of accounts, to file documents ,evidences etc for our behalf. Their actions and explanations will be binding on us.

We,xyz& co,being chartered accountants,duly qualified u/s 288 of the income tax act,1961,accept the above said assignment.

In kerala the stamp value is Rs.50/-.In other states depends on their own stamp act.
