Regarding pan card

29 August 2011 I lost my pan card & i have only a copy of latter.
But i used this pan card for my return filing
Than how can i Receive my duplicate pan card.

Q1 My old PAN Address is all so change & i want to give in my duplicate pan card.

Q2 What i do, can i file rectification form ?
