Fill the above form in Triplicate pest 1 rupees revenue stamp on advance receipt section. also send blank cancel check copy so that your pf amount will directly credited to your account
Submeet the form to your employer. Employer will verify your details and stamp it accordinlgy and send it to PF office. You can check the status of your claim on
