Fill "Request for changes/New PAN card form". Submit to your nearest NSDL TIN Faciliation centre.
Documents requiered: 1 colour 3.5 X 2.5 cm size photograph.
Prescribed ID proof & address Proof.

Fill a request for Changes or correction in PAN Card. And fill it with the same documents that you have attached to the original PAN Card.
In the last column of that application mention the PAN already allotted to you.
