1. One colour photograph 2. self attested copy of PAN card. 3. self attested copy of address proof and identity proof. 4. Declaration in prescribed format ,called as annexure-1. 5. Govt fee 100/- per application.
Din-1 is application form and it is compulsory. Din-2 is to intimate number to the Company and it is also compulsory. Din-3 is applicable / mandatory to those director's appointed before 1/7/2007 and used by the company to register the DIN in MCA portal.
