Please tell me wich Schedule the commition of insureane & Bank S/b A/c interest should fill in ITR4.

Commission from Insurance would be liable for tax under business Income head so it would be fill at Schedule BP and the interest from SB account is liable for tax under other income so it would be fill at Schedule OI.
