any one can tell me what procedure to be followed when income tax return has been filed but the bank has rejected the self assessment challan stating that name in pan card and in return differs.

you should kindly repay the tax and revise the return.
Another view can be that just pay the challan. As long as it is reflected in Form 26AS, credit for the same cannot be denied and your return should get processed.
