Please anwer this its really very urgent.......

Respected Sir/mam,
1.Please suggest me what are the documents required to be submitted along with the printout of the online reg. form because i havent seen anything mentioned there in ICAI's website?
2.Do i need to give my photo even after uploading it?
3.There is mentioned in ICAI's website to sign the form even after uploading the signature photo but the problem is where to sign i mean in the same box where the photo of my signature is ptinted?
4.After getting the printout there is a declaretion by the student do i need to mention there the details of completing my orientation and ITT progamme like date of completion and certificate no.
