27 January 2014 I got this remark from ROC-Kolkata, While Incorporating a company
1. Witness did not mention how many signature he/she witnissed.
2. Pls furnish copy of registered ownership deed and a title search report by a competent lawyer for registered office address.
3. Pls furnish an affidavit sworn before first class magistrate/notary from subscribers/first directors to the effect that proposed company/director shall not accept deposit unless compliance with provision of Companies Act 1956, RBI Act 1934, SEBI Act 1992 and rules / regulation made there under

27 January 2014 i) The witness has to write the following sentence in his/her own handwriting just above the witness's signature .
For example if there are three promoters write as follows.
Witness to the above three signatures.

ii) Documents to prove the ownership of registered office is a mandatory attachment to e-form 18. Therefore, please attach any of the above two documents prescribed.

iii) Format of affidavit is given below.
