31 October 2012 can anyone please provide the format of issuing shares for form FC-GPR. i would also require format of the undertaking of terms signed by cs...please help

Go to the master circular on FDI issued by RBI on July 02, 2012 in which Form-FC-GPR and CS declaration is attached.
