can an employer issue both form 16 and 18A to the same individual

theoretically, salary almost covers everything that is paid to an employee. So unless you are able to establish that a particular payment is out of definition of salary, you cannot issue Form 16A.

i am a professional doctor
i am on the rolls of state government and working as a govt doctor and drawing salary and getting FORM 16 Regularly
govt brought in a scheme under which i am paid part of the funds received by the govt hospital through a separate TRUST for that particular procedure.
THE DISBURSING AUTHORITY IS SAME AND HAVING SAME TAN UNDER WHICH HE IS DISBURSING SALARY AND ISSUING FORM 16 TO THE SAME INDIVIDUAL
i want to claim that payment as professional fee and want to take FORM 16A from the SAME DISBURSING AUTHORITY
PLEASE GUIDE WHETHER THE DISBURSING AUTHORITY IS RIGHT IN LAW

I believe that should be covered part of your salary only unless the govt itself wishes to keep it separate.
