I HAD THE FOLLOWING QUERIES:
1. THE CERTIFICATE NUMBER IN FORM 15 CB REFERS TO THE CERTIFICATE NO./REGISTRATION NO.OF THE ACCOUNTANT OR IS IT THE CERTIFICATE NO OF THE FORM ITSELF?
IN CASE OF LATTER, PLEASE QUOTE A SUITABLE EXAMPLE OR HOW THE CERTIFICATE NO IS TO BE FILLED?
2. WHAT SHOULD BE THE DATE IN FORM 15CB?

18 November 2013 u can quote any serial no./ certificte no. as a certificate no at ur own will, there is no prescribed format to quote the same.
as for as matter is concerned to 15ca then u have to fill it online at tin-nsdl site and it will generate a certificate no. itself.
