31 December 2011 what is procedure for filling din 4 for mismatching name of father as per pan card and din records? As as per roc records father name is Ravindrabhai Kamlesh shah and as per pan card father name is RavindraBhai K. Shah.Which proof i required to be attached?

Self attested copy of PAN Card.

02 January 2012 thanks for reply.but i m nt still clear as there is mismatch of father name, have i required to make correction in pan card and then only file din 4?

02 January 2012 If the name itself is wrong in PAN card, PAN database has to be changed first,get corrected PAN card ,then file DIN-4 along with copy of corrected PAN card. Other type of mismatch can be rectified through Annexure-2.
