Filing of form inc-22

03 January 2015 sir,
i did not file inc22 with inc7.now i have to file inc22 within 30days .i received incorp certificate.
while filing inc22 in item no 1 where shall i tick?
existing company or new company?
if i tick existing company
what about item 3(d) Purpose of the form?

please clarify
