16 February 2015 My client has purchased some goods from Interstate dealer and applied for C form. But some how before issuing it to dealer only he lost all c forms. Amount is heavy.

Please let me know what is the procedure for getting duplicate C form. If C Forms are not issued my client has to pay tax. Please tell me procedure for duplicate C form application along with required documents list or formats of any.

My client belongs to Maharashtra, Aurangabad.

16 February 2015 I think first the concerned person has make a DDR in police station against the lost of C form (Containing C form Nos) then same has be publish in new paper along with notification.

After this the both doc (DDR and New Paper) have to be submitted in the Sale tax office.. then they will issue the new C form...

But ask the correct procedure from your sale tax officer

24 February 2015 you can contact to the sales tax officer. first of all you have to lodg the fir with C forms Sr. numebr & nos of form after that. deptt. isse the form after taking security against duplicate form.

24 February 2015 Requirement paper if Form-C Lost
1. Intimate your sales tax office for the lost of "C" form.
2. Intimate your purchaser and give the following documents
for issuing duplicate "C" form.
3. Indemnity bond (form G), Indemnity Bond on None judicial
Paper. (The prescribed form of Indemnity bond is in Form G.)
4. Advertisements copy one should of English and one should
in local language for lost of "c" form giving full
description of "c" form. Lodge an FIR, get it published it
in local newspapers and informs the sales tax authorities.
5. FIR copy of lost "c" form.
6. Request for issuing duplicate "c" form to your
purchaser. (All Invoice Photocopy & GR Photocopy with above
documents submit to department and get other C Form.)
