Din 4 director declarations pls help

i have to prepare declarations for directors for filing form DIN 4




in my case I have to intimate PAN card no. of directors for the first time...so my query is whether the format as per MCA holds good or i have to use word INTIMATION instead of CHANGE and in some cases i have to intimate PAN of directors and also the changes in director's address......so in those cases can i use like this INTIMATION / CHANGE......


is this ( intimation / change ) right or i have to use the word change only for intimation of PAN as well as change in director address ?

30 August 2011 1) For PAN only use the word 'intimation' and strike off 'change'.
2) For PAN intimation and change of address, use the words 'change / intimation'.
