Please tell me how can I get Digital Signature for submitting Income Tax Return?

Apply through the agency for a PAN based DSC.
Requirements are:-
a) one photograph.
b) PAN card copy.
c) address proof
d) Fees. varies for 1 / 2 year duration, with/without usb token etc.,
