Change in object clause under companies act, 2013

28 May 2014 I have filed Form MGT-14 for change in object clause, but ROC replies as under :

"according to sec 13(8) of companies act 2013 ,please give declaration that company has not raised money from public through prospectus and still has not any unutilised amount out of the money so raised,corresponding to previous objects of company. Form 67
(Addendum) should be filed by 12/06/2014 failing which the eForm shall be treated as
invalid and shall not be taken on record. (Please refer Regulation 17 of the Companies Regulation, 1956)"

please send the format for such declaration.
