whether I T service provider can issue c form for capital goods purchase?

Yes, if such capital goods are mentioned in his CST registration certificate.

normally IT service provider is not resgistered in sales tax
please check
but if they are selling software then they must be, as software selling is also included in sales tax
now check whether these items are included in their certificate or not
if yes, only then they can issue
