Urgent:Plz suggest and help me prepare my bio-data for formal application to CA firm

my relative who has shortlsted some CA firms for articleship, today asked me to send him my bio-data/resume through email.
Sir, plz suggest me

a)what points should i include in my resume?

b)What should be the format?

c)an illustrated example of resume;

TIME PERIOD FOR ITT TRAINING.

SIR,
I WANT TO ASK THAT WETHER THERE IS ANY RULE THAT ITT TRAINING MUST BE COMPLETED WITHIN 6 MONTHS AFTER GETTING ARTICLE SHIP IN PCC.
PLEASE ANSWER AS SOON AS POSSIBLE, THANKS FOR YOUR RESPONSE.

when my PCC attempt will due

I have passed my PE I exam in November-2006 but I have applied for provisional registration for PE II after giving PE I examinatin in May-2006 but I was failed in that attempt.My articleship is commenced from 18th January, 2007. I have also taken tranfer from PE II to PCC. Now I want to know that when my first PCC attempt will due?

why is this current trend

by CA firm preferring PE II students over CPT?and why will that happen till July 2008?

If that had to happen, why was cpt introduced?

articleship

Is it good to join big firms/ or small firms?i know the +'s and _'s of both.keeping in mind the study schedule,will any medium sized firm fit for doing articleship? big firms though add a lot to our C.V.? pls advice.I'm confused!! i got 79% in CPT.
