Dear Friends / Experts,
Please guide me on the following, IF POSSIBLE:
For effecting Address Change for APPLICANT (INDIVIDUAL) in Form ST-1, I am attaching Copy of DL, Amended Form ST-1, Acknowledgement Receipt and Old Form ST-2, duly signed.
Query
Whether the APPLICANT (INDIVIDUAL) should also attach his signed Copy of PAN Card in addition to the above documents.
Kindly confirm, please..............

No need to submit PAN copy again for amending address, address proof is enough along with the document you have mentioned
