Termination of articleship after 1yr.

I m in 2nd year of ca articleship now i want to terminate from my present auditor due to some health problem.should i intimate the same to my auditor before sending the medical certificate to ICAI.I want the procedure and documents required to get terminate. i want to know the detailed procedure.

Is medical certificete must be signed by the auditor.but when i enquired in the icai chennai,they told me to post the medical certificate and a letter mentioning the reason for termination thats it.if i m wrong plz tell me what is the correct process.
