At the time of deduction on salary is this mandatory for employer to look/take all evidence of deduction (under chapter vi) claimed by his employee.

Yup dear, its mandatory for the employer to collect all details from employee.

you have to deducted tds when you make payment of salary. and also take evidence of chapter iv.
