If any employee's salary is more than Rs. 1.50 lakh only then you need a salary slip to bifurcate the salary in diff. parts. However you can use the below heads:
Basic salary
H.R.A.
Reimbursement of Medical Expenses
Transport Allowance
Uniform Allowance
Children Education Allowance
