Revised itr not uploading

I had filled the Original ITR for AY 2011-12 on 02.05.2011 and now I am filling REv.Itr but it is not uploading at ITD site , it showing below message :
If your Return filed under section code is 139(1),139(4),142(2),148,153A then your return type should be Original(O) from AY2011-12 onwards.
Please help me how to upload rev. itr

you have not sent ITRV to cpc or the acknowledgement number is wrong
