my pan card is damaged. what's is the procedure for replacement of the card with same pan card number.?

Thereafter select 'changes or correction in Pan' option.
While filling this form, applicant should not select any of the check boxes on the left margin of the form.
After filling submit it with requisite fee of about Rs 96 for reprint.
You can also opt for manually filling of this form by downloading from the same site and submit it to any nearest TIN faciallation Center.
