Please help me in filling the MEF 2011-12 (Bank Branch Empanelment Form)

MY DETAILS ARE AS UNDER:
I have Qualified & taken COP in year 2008, till mid of 2009 i was partner is some other CA firm and thereafter create my own Sole Prop. Firm.

while filling MEF 2011-12 form, at first page "Details of the Applicant" there are 2 (two) option showing with CHECK BOX to tick on any one:

1. Yr. of Estb. of Sole Prop. Concern
2. Yr. since continuously holding COP in
Individual name.

Query:
1. In which option i have to tick (Please suggest best)

2. What is the purpose of showing above 2 (two) option in MEF??

Please guide as early as possible as there is not much time left for filling MEF.
