Lost my form no. 109

05 September 2017 I have lost my form no.109(which was submitted to the ICAI @ the time of transfer) required to be submitted for Re-registration of Articleship. Will it be ok if my new principal CA write a letter to ICAI ? WHAT MAY BE THE APPROPRIATE SOLUTIONS?
